Conscious Change uses the ChARM Patient Portal. ChARM is an Electronic Medical Records System. Existing patients have the ability to communicate with us and access health records via the secure ChARM portal.

Conscious Change does not schedule any appointments via ChARM. To make an appointment, please call us at 662.638.3388.
